Moving Forward Together

Community Case Management

One-on-one support to help you get back on your feet and move forward.

Every day, we welcome individuals, families, and older adults facing diverse challenges – financial emergencies, hunger, unemployment, housing, instability at home, and growing concerns with physical or mental health. When you are working hard, but falling short, case managers serve as committed partners in navigating the resources available to you and your family.

First, we listen. Then, we work together to problem-solve, find the most helpful options for your unique situation, and create a plan to support your immediate stability and long-term goals.

Take the Next Step

“There are a lot of good people out there, but to me you really shine…going above and beyond, in such an enthusiastic way. Life gets so hard for people, and you offer such kindness and hope with what you do.”
