An integrated support system for families impacted by Alzheimer’s Disease and Related Dementias (ADRD)
When it comes to coping with Alzheimer’s, every journey is different. That’s why, in partnership with Alzheimer’s San Diego, we’ve built a care network designed to wrap around the unique needs of individuals and caregivers impacted by ADRD. Whether you’re concerned about a loved one or coping with the symptoms of dementia on your own, we’re here for you and your family.

Care Planning & Management – With specialized training in dementia care, JFS Care Managers offer straightforward information, a personalized connection to resources and support, and a clear line of communication between clients, family members, and health providers.
Balboa Avenue Older Adult Center – Join our friendly group for lunch and a full menu of activities designed especially for members of our community living with early-stage Alzheimer’s and related dementias.
Monday, My Way – A specialized day program at our Balboa Avenue Center, free for people living alone with ADRD. Participants receive a “full service” check in with a JFS Care Manager, who can monitor for health and safety concerns and connect you to additional services.
Music & Memory – An in-home music therapy program designed to help manage behavioral symptoms and anxiety, while fostering a greater sense of connection and well-being.
Out & About – Join us for Friday adventures to points of interest across San Diego. Every trip is led by our professional staff and includes lunch at a local restaurant or deli. Transportation is provided by On the Go, our award-winning transportation service for older adults.
Caregiver Support – Connect to a range of affordable options for respite, behavioral symptom management, and home-based support. We also offer bi-monthly support and discussion groups just for caregivers, facilitated in partnership with Alzheimer’s San Diego.
