Moving Forward Together

Family Support Liaison Program

Doing whatever it takes to support your family

Building a healthier, more resilient San Diego starts with supporting our community’s youngest members by strengthening families.

Our free program uses a family-centered, culturally responsive approach aimed at helping families set and achieve their own goals. This program supports Black and African American children and families. By applying the C.A.R.E.S. (Coordination, Advocacy, Resources, Education and Support) Wraparound model, we partner with families to design an individualized support plan that works best for them.

Our prevention and family preservation strategies help parents and caregivers navigate the child welfare system, connect to community resources, and increase their networks of support—all with the goal of keeping children safe and families together.

Together, we will:

  • Determine what is needed to help manage and eliminate the stressors.
  • Build a Family Team, consisting of other providers and natural supports.
  • Identify what is needed to help each family be successful.
  • Follow the process called “Wraparound Family Support” which means that support and services are “wrapped around” each child and family to sustain.
  • Find creative solutions based on each family’s strengths, needs, culture, values, and community.
