At JFS, we’re committed to meeting the changing needs of older adults as they age. Our team of trained aging professionals act as advocates, consultants, and guides—offering wide-ranging expertise on issues related to aging. We have specialized training in dementia care and are adept at working with out-of-town families to coordinate long-distance care.
Aging Care Managers conduct a thorough assessment of an individual’s needs and strengths, provide appropriate referrals/resources and create a custom care plan meant to serve as a guide to empower the individual to reach their care goals.
With a personalized care plan in place, our Aging Care Managers and Aging Care Associates will work together to provide in-home support and care monitoring. As needs and circumstances change, we adjust to help you maintain your health and wellbeing. With your consent, we can also be the “eyes and ears,” communicating with your care team on a regular schedule or as needed. Examples of things we can help with are:
