Do you want to stay in your own home, but worry you can’t do it alone? Could you use a little assistance making doctor’s appointments, organizing affairs, or accomplishing everyday tasks? Are you are overwhelmed with the responsibilities of caring for a loved one? Do you need a trusted set of eyes and ears for the times when you can’t be there yourself?
“You have given our family such peace of mind. I love knowing that JFS is there, just a call away, ready to help.”
At JFS, we’re committed to meeting the changing needs of older adults as they age. Our team of trained aging professionals act as advocates, consultants, and guides—offering wide-ranging expertise on issues related to aging. We have specialized training in dementia care and are adept at working with out-of-town families to coordinate long-distance care.
Aging Care Managers conduct a thorough assessment of an individual’s needs and strengths, provide appropriate referrals/resources and create a custom care plan meant to serve as a guide to empower the individual to reach their care goals.
With a personalized care plan in place, our Aging Care Managers and Aging Care Associates will work together to provide in-home support and care monitoring. As needs and circumstances change, we adjust to help you maintain your health and wellbeing. With your consent, we can also be the “eyes and ears,” communicating with your care team on a regular schedule or as needed. Examples of things we can help with are:
An in-home music therapy program designed to help manage behavioral symptoms for persons living with Alzheimer’s Disease or related dementias, while fostering a greater sense of connection and well-being.
Are you overwhelmed, frustrated or just unsure of what to do? There are a lot of pieces to the puzzle of aging. Get some clarity and talk things through with one of our Aging Care Managers to receive support, advice and guidance to making sense of these puzzle pieces.
Connect to a range of affordable options for respite, behavioral symptom management, and home-based support. We also offer bi-monthly support and discussion groups just for caregivers, facilitated in partnership with Alzheimer’s San Diego.
Find friendship, relief, and understanding at our bi-monthly Support & Discussion Group, delivered in partnership with Alzheimer’s San Diego.
Home safety plays a significant role in allowing older adults to age in place. Our dedicated volunteers can help with minor home-safety repairs and modifications at no cost to you or your family.
