Do you want to stay in your own home, but worry you can’t do it alone? Could you use a little assistance making doctor’s appointments, managing medications, or accomplishing everyday tasks? Are you are overwhelmed with the responsibilities of caring for a loved one? Do you need a trusted set of eyes and ears for the times when you can’t be there yourself?
At JFS, we’re committed to meeting the changing needs of older adults as they age. Our team of Aging Specialists act as advocates, consultants, and guides—offering wide-ranging expertise on issues related to aging. We have specialized training in dementia care and are adept at working with out-of-town families to coordinate long-distance care.
“You have given our family such peace of mind. I love knowing that JFS is there, just a call away, ready to help.”
As Aging Specialists, we work closely with older adults and their caregivers to assess individual needs, navigate a range of options, and put the right plan in place to enrich our clients’ lives. Through thoughtful conversation and observation, we’ll create a custom care plan, looking at 13 areas of wellbeing.
With a personalized care plan in place, our Aging Specialists and Case Aides work together to provide in-home support and care monitoring. As needs and circumstances change, we make adjustments to help you maintain your health and wellbeing. With your consent, we can also be your family’s “eyes and ears,” communicating with them on a regular schedule or as needed.
In partnership with Alzheimer’s San Diego, we’ve developed a comprehensive support network, with services tailored for households impacted by Alzheimer’s Disease and Related Dementias (ADRD).
